Displays and handles Web questionnaires
Mod_survey is not developed anymore and the homepage of the project is only available through archive.org.
Mod_survey was an Apache mod_perl module which allowed users to create their own Web questionnaires using an XML-based tag notation. It supported exporting of data into several file formats, including SPSS syntax, semi-colon delimited fields, and SQL script. It also provided limited support for descriptive statistics of the submitted data, and stylesheet customizations of layout.
